Liability Release.

By clicking "I Agree" or checking the box below, I voluntarily agree to participate in sound healing services provided by Sound Garden or the sound practitioner, Minh Tran. I understand that sound healing involves the use of various sound frequencies and instruments, including, but not limited to, singing bowls, tuning forks, gongs, and other instruments, and that these services are offered as a complementary and holistic approach to well-being.

1. Acknowledgement of Risks

I acknowledge that sound healing is not a medical treatment, nor is it intended as a substitute for professional medical or psychological care. I understand that results may vary from person to person and that there are no guarantees of specific outcomes. I acknowledge that sound healing may not be appropriate for individuals with certain medical conditions or health concerns, and I have disclosed any relevant health information to the Practitioner.

I understand that the Crystal Sound Energy Healing session, while rooted in ancient and historical origins, is a modern, experiential modality within the healing arts. The work I receive is intended for the purpose of relaxation, stress reduction, personal growth, spiritual support, and establishing peace of mind. I am participating voluntarily and acknowledge that the work is not a substitute for medical examination, diagnosis, or treatment.

2. Voluntary Participation

I understand that during the session, repressed or dissociated memories of past trauma may surface. I acknowledge that the practitioner, Minh Tran, will not support the claim that these memories are/are not real and will not testify to such a reality in a court of law. However, I understand that these memories, if they surface, may be of significant benefit in working to resolve issues related to those memories.

I understand that it is my responsibility to communicate any discomfort, emotional reactions, or concerns during the session. I agree to listen to my body and speak up if I feel any distress, and I can choose to withdraw from the session at any time.

3. Release of Liability

I hereby release and hold harmless Minh Tran, Sound Garden, and all associated personnel, agents, and family members from any and all liability, claims, demands, or causes of action, including but not limited to personal injury, emotional distress, or any other harm arising from my participation in the sound healing session, whether such injury or harm occurs during or after the session.

I understand and agree that the services provided by Minh Tran are not intended to diagnose, treat, or cure any medical or psychological conditions. I affirm that I have consulted with a healthcare provider if I have any concerns regarding my participation in the session. I also affirm that I have disclosed any health conditions, and I agree to inform Minh Tran if any of my conditions change.

4. Medical Disclaimer

I have been advised that the “Alchemy Sound 1-on-1 experience” should not be construed as a substitute for medical examination, diagnosis, or treatment. I understand that if I am suffering from any mental or physical ailment, I should seek the advice of a physician, chiropractor, or other qualified medical professional. Minh Tran has informed me and I am aware that Sound Garden is not licensed to practice any form of medicine under the laws of this state.

5. Confidentiality and Media

I confirm that no recorded transcript, transmission, or videotaping is being taken of any part of this session or any conversation occurring during this session, unless otherwise agreed to in writing. I understand that my personal health and session details will be kept confidential and only shared as required by law.

6. Acknowledgment of Terms

By checking the box below, I acknowledge that I have read and fully understand this Liability Release and Waiver. I agree to the terms and conditions outlined above and accept full responsibility for my participation in sound healing services.
